How are health plan costs traditionally split between employee and employer?

Employers are typically required to pay at least 50 percent of an employee's monthly premium in order to qualify for "group" status with the health insurance carrier. These percentages, however, can vary from plan to plan or from employer to employer. Carriers usually don't require the employer to cover the premium costs of dependents, but employers often do, anyway, as a benefit.
